Waiver (electronic signature required)

As a parent or legal guardian, I agree and consent to having the staff members or teachers of Rosemont Baptist Church to secure any treatment that may be necessary for my child. I hereby waive and release Rosemont Baptist Church from liability and will be responsible for any medical or other type of charges in connection with my child's attendance and/or participation in Rosemont's children's or youth programs and activities. I also agree to allow pictures and/or video to be taken and used for publicity purposes by Rosemont Baptist Church. This also covers any mini-field trips taken during Vacation Bible School for 5th-6th grades.

Photo Release

By signing this field, I give Rosemont Baptist Church permission to photograph my child and use his or her pictures in the church’s digital and print communications. Rosemont Baptist Church will never publish a child’s name or any other personal information with any of its publications. By not signing this field, you indicate that you do not give permission for Rosemont to use your child's pictures in the church's digital and print communications.
